*Our office must diagnose all treatment eligible for the TDAH Membership discount. Family members cannot be substituted for another family member who was not originally a member. This membership is Non-Refundable. Exams, x-rays, cleanings, and fluoride must occur within the year of enrollment and cannot be carried over to the next year. No refunds will be given if patient chooses not to use the member- ship, even if moving out of state. Your effective date is the day you sign up and your renewal date is the same date every year. This membership expires after 1 year. Co-payments for treatment are due at the time of service. Rates are subject to change annually. Our plan is valid only at TDAH; therefore if you are referred to a specialist, discounts offered to members may not apply. A fee (based on length of appointment) may incur for each broken appointment without a 24-hour advance notice. TDAH Membership is not insurance and it is not a discount dental plan. TDAH Membership is a patient loyalty program only and cannot be combined with insur- ance plans. TDAH Membership is not a risk-bearing insurance model. There are no fee-splitting arrangements between TDAH and our specialists. TDAH Membership is not transferable to another dental provider.
